Nose cowl can be split into two halves and can be transported in a normal freighter.

Today, Emirates Boeing 777-300ER registration A6-END departed from Islamabad at 9:14 AM as ferry flight EK-2514 to its home base Dubai where it landed at 11:13 AM completing flight in three hours.

The B777 was damaged by Gerry's dnata ambulift at Islamabad Airport on February 27.

A6-END resumed performing revenue flights for Emirates when it departed from Dubai today at 1:02 PM as flight EK-378 to Phuket.

Causes / Contributing Factors

Cause


The operator lifted the van box of ambulift to the height of aircraft door without extending arm-braces, which caused ambulift to become unbalanced and toppled.

Note: Aviation Occurrence Category (ADREP Taxonomy)-Ground Handling (RAMP)

Contributing Factors

The design safeties provided to restrict operation of the van box without extension of arm-braces were not functional.

Unserviceability of system designed safeties and re-wiring of relay switches remained undocumented and unrectified during inspections by Ground Handling Agent (GHA).

Marshaller was not available and operator installed the ambulift with aircraft without any assistance/marshaller.

The operator was not authorized and qualified to operate ambulift.

Pakistan Civil Aviation Authority (CAA) oversight was not effective to ensure compliance to relevant

Standard Operating Procedures and Airworthiness Notice by the GHA.

Looks like DNATA local franchisee is by passing all controls. Any fines imposed on Gerry's DNATA?

In the photos of toppled vehicle it can be seen that the outrigger stabilizing feet were not deployed.

i was thinking it might be hydraulic failure of the jacks but seems that pre-requisite setup and stabalisation process was ignored and also technical interlocks designed to mitigate human factor failings were also bypassed without someone understanding the consequences.
