Because people take bus or trains. Do you expect people to go airport 2 hours before departure, then 1 hour after arrival etc for a journey that can be done in 5 hours? Even then, the flight will be only once a day. India is a billion plus population. If 20% uk doctors are indian origin, it is because of huge indian population, and for example, they're like 5% of total indian population. If 20% doctors were Pakistani, they would be for example, 50% of pakistani population.
India is a big country and they have far more airports. In Pakistan, there are only 5-8 major airports which are very close that people are fine taking bus/trains/car and fly only when going international

This is one reason ME3 airlines are successful. They gather all the people from all over the world to faislabad in one aircraft daily. Meanwhile PIA cannot do faislabad to London, Manchester, Australia etc, forget daily flights. Only solution is connection which is really bad if domestic, so people choose me3.
What is needed is high speed trains (not bullet) which are very good so people can take those with a stop at airport. Pakistan is better suited for trains apart from some airports.
